Backend & Infrastructure Intern | Pear

Remote | Summer/Fall 2026 | 10-12 weeks (potential extension)

What we do

Pear builds cutting-edge AI for healthcare. We’re solving patient acquisition and provider discovery with systems that actually reason about medical complexity.

Pear Care is rewiring how patients and providers connect. We build intelligent healthcare infrastructure that takes the friction out of medical workflows—from analyzing patient symptoms to matching them with the right specialists and automating the scheduling process. Our systems parse, validate, and route highly complex clinical data in real-time, operating under strict security guardrails because in healthcare, precision is non-negotiable.

Our team is small. Engineers come from Google, Microsoft, and Visa. We ship weekly.

The role

You’ll own real systems, not side projects. We need a hardcore backend and infrastructure engineer who can build resilient services and scale our AWS environment. Accuracy matters, and latency is felt.

Full autonomy. Pick your tools, design the architecture, ship it.

What you’ll build

Agentic Backend & API Design. Build scalable, event-driven architectures. You’ll design the core APIs that orchestrate our multi-agent workflows, managing persistent agent state machines, tool-calling loops, and WebSockets for real-time data streaming to the client.

AWS Infrastructure & DevOps. Take ownership of our cloud environment. You’ll provision, manage, and scale our AWS infrastructure (ECS, Lambda, RDS, API Gateway) ensuring high availability, secure VPC configurations, and optimized compute for heavy AI workloads.

Clinical Data Pipelines. Build robust data ingestion and processing systems. You’ll write the logic that securely handles, validates, and routes highly complex medical data, ensuring our backend flawlessly processes schemas involving ICD-10, CPT, and NDC codes without dropping packets or failing silently.

System Reliability & Security. Design fail-safes and redundancy for high-stakes healthcare environments. You’ll implement robust logging, observability, and data encryption practices to ensure our infrastructure remains compliant and highly available.

Performance & Optimization. Monitor, profile, and optimize the entire stack. You’ll hunt down latency bottlenecks—from database query execution times to time-to-first-token (TTFT) across the LLM pipeline—and implement caching layers, connection pooling, and rate limiting.

Who we’re looking for

You’ve shipped something real. Not tutorials, not notebooks. A deployed backend, API, or cloud infrastructure that handled real traffic or real users.

Strong Node.js, TypeScript, and Python. You care about async/await patterns, event loops, and writing robust server code, but you’re also comfortable diving into Python for our AI and data workflows.

AWS and Cloud-Native fluency. You know your way around IAM roles, serverless deployments, database scaling, Bash scripting, and infrastructure-as-code (Terraform/YAML).

Database & Systems thinking. You understand how to write complex SQL queries, design scalable database schemas for clinical data, and manage complex migrations.

Founder mindset. You find architectural bottlenecks and fix them without being asked. You treat the codebase like your own company.

Compensation

$4,000 to $6,000/month depending on experience

Paid bi-weekly

Full-time, 40 hrs/week
